Summary
Glaucoma, as a condition requiring lifelong follow-up, has long faced challenges such as limited medical resources and low patient awareness. To address these issues, our institution established the first guideline-based, “de-expertised” glaucoma case management model in China, along with a self-developed intelligent data management platform integrating functions including follow-up appointment scheduling, reminders, disease progression analysis, and health consultation.
This model has been implemented for over a decade and has managed more than 1,800 glaucoma patients. Currently, the target intraocular pressure (IOP) setting rate reaches 92.1%, and the target IOP achievement rate is 68%, meeting international leading standards. The establishment of individual patient case files has strengthened the doctor–patient relationship and enhanced patient loyalty to the hospital.
Building on this foundation, our institution, as the initiating organization, has collaborated with over 100 hospitals nationwide to launch a series of large-scale public education activities, effectively increasing public awareness of glaucoma.
