  2. Better pathways, not just more services.
  3. Late eye care means lost vision.
  4. Good governance beats legal structure.
  5. Measure value, not just volume.

Summary

SUS is one of the world’s largest universal healthcare systems, built on the principle that health is a right of every citizen. São Paulo, with 46 million people in the state, shows how hard it is to turn that right into real care.

The main challenge is not coverage but access. In eye care, long waits can mean permanent vision loss, so we need better care pathways, not just more services.

Nonprofit partnerships and specialized hospitals can expand access, if governance is strong and incentives are aligned. The way forward: integrate primary and specialized care, use technology, and measure value, not just production.
