Delivering high-quality cataract surgery amid growing demand requires more than surgical excellence – it requires an efficient, patient-centered operating theater workflow.

This presentation describes our approach to optimizing perioperative processes, standardizing clinical pathways, and enhancing multidisciplinary coordination to achieve high-performance cataract care. By improving workflow while maintaining quality and safety standards, the initiative demonstrates how high surgical volumes can be achieved without compromising clinical outcomes or the patient experience. The approach is grounded in the principle that every individual with avoidable cataract blindness deserves access to a decent standard of care. Operational efficiency is therefore not only a management objective but also a mean of expanding equitable access, promoting sustainability, and maximizing the impact of limited healthcare resources. As part of this work, an innovative study of value-based metrics in public eye care in Brazil aims to integrate clinical outcomes with patient-reported outcome measures (PROMs) and patient-reported 2 experience measures (PREMs) to provide a more comprehensive assessment of the value delivered to patients.
