Bridging the Knowledge–Behavior Gap: A Behavioral Approach to Improving Incident Reporting in an Eye Hospital

Summary

Background: Under-reporting of incidents and near-misses remains a major barrier to patient safety in eye care because knowledge alone does not reliably translate into reporting behavior.

Objective: To improve incident and near-miss reporting rates among staff in a specialized eye hospital through behaviorally informed interventions.

Methods: A prospective quality improvement project was conducted using a Knowledge–Attitude–Behavior (KAB) survey and behavioral barrier analysis to identify obstacles to reporting. Interventions were designed using the BJ Fogg Behavior Model and implemented through Plan–Do–Study–Act cycles. Strategies targeted motivation, ability, and triggers through a non-punitive policy, simplified QR-based reporting, near-miss training, and regular feedback.

Results: Baseline survey showed high knowledge regarding incident reporting (mean 4.29/5), yet only 16.5% of staff had submitted an incident report in the previous six months. Following implementation, incident reports increased from 24 to 35 over three months (+46%), while near-miss reports increased from 12 to 32 (+166%).

Conclusion: Addressing behavioral barriers rather than knowledge alone substantially improved reporting and fostered a stronger learning-oriented safety culture. Combining the KAB framework with behavioral science offers a practical, scalable approach to strengthening reporting culture and organizational learning in eye hospitals.
